A patient-centered approach was used to develop an energy-returning mechatronic ankle foot orthosis (AFO), called ADJUST. ADJUST was designed to provide independent support for flaccid plantarflexor and dorsiflexor muscles without limiting normal ankle range of motion (ROM). Mechanical and clinical evaluation showed that ADJUST was able to meet the most important stiffness-, kinematic- and kinetic- requirements. When compared to patients’ own AFO, beneficial effects of ADJUST on ankle ROM and patient satisfaction were most prominent during controlled plantarflexion and when walking downhill. To fulfill all requirements, the weight and size of ADJUST should be decreased, and individual automatic adjustments of its initial alignment and stiffness configuration should be realized. With ADJUST, an AFO was developed that is suitable for a large group of patients. This assumption was enhanced by the fact that a large partner in the industry: Otto Bock showed interest in ADJUST and invested in a patent.
